 c: Stroke Team Simulation for Transitions of Care is a virtual simulation
  in which teams of advanced-level students from different health professi
 ons work together to coordinate care and develop a discharge plan for a p
 atient who has experienced a stroke. This simulation is led by the HIPE C
 enter and was developed by a task force representing 5 professions with c
 onsultation from an expert panel representing 9 professions.\n\nTeamSync 
 is held during the fall and spring terms each academic year. Learner self
 -assessment data indicates that outcomes are being met across assessment 
 domains. This capstone experience is the most advanced level of the HIPE 
 Center’s Health IPE Microcredential Program and participants will earn th
 e Transitions of Care microcredential upon completion of all requirements
