Alcohol-Associated Hepatitis and Early Liver Transplantation

About

OverviewThis activity reviews the diagnosis and management of alcohol-associated liver disease (ALD) and alcohol-associated hepatitis (AAH), including alcohol cessation strategies, treatment of alcohol use disorder, appropriate use of corticosteroids, and the evolving role of early liver transplantation. Learners will gain practical guidance on risk stratification, treatment thresholds, and timely referral of patients with severe alcohol-associated liver disease.

Target AudienceSpecialties - Family Medicine & Community Health, Gastroenterology, Gastroenterology & Hepatology, Hematology, Internal MedicineProfessions - Advance Practice Nurse (APN), Non-Physician, Nurse, Physician, Physician Associate, Registered Nurse (RN) ObjectivesAt the conclusion of this activity, learners should be able to: 1. Apply NIAAA clinical criteria and phosphatidylethanol testing to accurately diagnose alcohol-associated hepatitis and distinguish alcohol-associated liver disease from MASLD. 2. Use MELD rather than the Maddrey discriminant function to risk-stratify patients with alcohol-associated hepatitis and identify candidates for corticosteroid therapy. 3. Manage corticosteroid therapy appropriately, including assessment of contraindications and calculation of the Lille score at day 4 or 7 to discontinue treatment in non-responders. 4. Initiate concurrent treatment of alcohol use disorder using behavioral support and evidence-based pharmacotherapy, avoiding agents contraindicated in liver disease. 5. Identify patients appropriate for early liver transplant referral without applying a fixed 6-month sobriety interval, and document eligibility using specific, non-stigmatizing language.
